Many people like to have a contact form on their site for feedback. This one is quite simple.Grab the zip file.  Contact

Inside are two files. the first is the html file. Just CCP everything starting with <form> and ending with </form> and put it on your webpage. You may adjust the size of the textboxes, put it in a table or use other "window dressing".

The second file is send.php. Go into the edit and replace me@mydomain.com with your E-mail address and save. That's it. You have a contact form.
